Fix two task-workflow bugs: map-requirements spec.md path and finalize-tasks create_intent error hint
Two bugs block the standard map-requirements → finalize-tasks workflow. The P1 bug causes map-requirements to look for spec.md in the coordination worktree (where it does not exist) instead of the primary checkout. The P2 bug leaves the finalize-tasks --validate-only error for planned-new-files without an actionable YAML example and without regression test coverage, allowing silent re-regression.
