Mission Run: Accept fail-closed on absent lanes.json
Make 'spec-kitty accept' fail closed when lanes.json is absent instead of silently skipping the acceptance-matrix gate (#4891).
Fixes P0 #4891: accept currently exits 0 and records/commits acceptance on an unverified matrix whenever lanes.json is absent, because the lane-gate resolver treats a genuinely-absent manifest as a silent no-op. The fix mirrors the existing corrupt-lanes branch so absence is fail-closed, consistent with implement/review/merge which already refuse via MissingLanesError.
