Mission Run: Accept fails closed on a stale or pending acceptance matrix
spec-kitty accept and orchestrator-api accept-mission never record acceptance over a pending, failing, or erased acceptance-matrix verdict.
Closes the P0 residual of #4858 (#4974): accept writes its pre-check matrix snapshot back without a lock and can erase a concurrently committed failing verdict. Makes the locked re-read/splice write the single acceptance-matrix write seam (#4887) and has orchestrator-api accept-mission apply the same readiness verdict as host accept (#4934).
