Consolidate lands only the work a reviewer approved, and says verified only when that is true.
Today a commit added to a lane after its work package was approved lands on the target with a verified banner, so unreviewed code can reach the target unnoticed. This mission makes the commit recorded at approval the single reference for what review approved: later commits are refused with a named error and a re-review remedy, approvals with no recorded commit fail closed with an operator attest path, and the verified banner is printed only when the landed content is the approved content.
