Mission Run: Coord Reads Fail Closed
Stop an unmaterialised coordination surface from silently overwriting or hiding a teammate's real mission files during ordinary coord-topology commands.
On a coord-topology mission from a checkout where the coordination worktree is not materialised (a fresh clone, a CI runner, a removed worktree), two commands read through an empty or status-only coordination surface and then act on it as if it were the real document: tracer-append rewrites a traces file from an empty default (destroying every earlier finding, exit 0), and the decision ledger resolves to a status-only husk that blocks a mission's acceptance with no CLI way out. This mission makes those reads fail closed against the authoritative file instead of clobbering or split-braining it. Closes #4959 and #4966 (siblings under epic #5002).
