Fix four accepted P0 defects where a read, compute, or report step acts on a partial, pre-merge, or recompute-from-scratch view of persisted state.
Four independent P0 bugs each let a downstream step trust the wrong slice of state: a scaffold placeholder poisons an acceptance verdict, a retrospective reports pre-merge counts, a failed upgrade wedges a project by stripping its schema stamp, and a finalize re-run clobbers planning provenance. Each already has a committed red-first reproduction. This mission remediates all four as independent, surgically-scoped fixes so operators can accept missions, trust retrospectives, recover broken upgrades, and re-finalize plans without silent corruption.
