Refuse a duplicate mission create (same slug+type, not abandoned) with an actionable error and an --allow-duplicate escape hatch.
spec-kitty specify / agent mission create invoked twice for the same intent silently creates two missions (both exit 0); add a fail-closed idempotency guard with an abandonment-aware auto-allow and an explicit override for the legitimate second-mission-same-name and factory-volume cases (#4033).
