Mission Run: Persistent deactivation: one activation ledger
A deactivated charter element stays gone: one activation ledger holds allow and deny state, and cascade, upgrade and presets honour it.
Core side of spec-kitty-charter ADR-0013 (#62 #63 #64 #66 #67). Today deactivation only removes an id from an allow-list, raises when the list is absent, and is undone by cascade, upgrade resets and org required_* unions. A new .kittify/charter/activation.yaml becomes the single authority for activated_<kind> and deactivated_<kind>; cascade skips and reports excluded nodes; deactivating a required artifact is refused naming the org pack.
