Make four consolidate/doctor safety fixes that rc5 claimed actually hold.
Release testing showed four rc5 consolidate fixes do not fully hold: canceled dependency-lane code still ships, a resume remedy drops a lane, coordination teardown destroys a late status commit, and doctor --fix reverts a reviewer reopen. This mission closes those four shapes so operators can trust consolidate's exit status.
