Stop any agent from pulling a work package out of review or approval without a recorded verdict, and repair the review-cycle surfaces that break after it happens.
agent action implement silently force-moves a WP out of for_review, in_review or approved with a fabricated rework reason (#5446, P0). The forced exit then deadlocks every later approval. Fold in the single implementer-of-record authority (#5340), the forced-approval restamp residual (#5721) and the review-cycle-1 overwrite (#5194).
