Close three exit-0 silent-data-loss defects (#4908 charter recompile, #4897 mission-state --fix, #4894 merge-driver-traces) by reading the SSOT / preserving-unless-proven-redundant.
Three independently-reproduced QA P0s share a family: a documented operation performs a destructive write on healthy input, returns exit 0/errors 0, and warns nothing. Fixes are seam-respecting per-issue (SSOT read; shared authoritative-event-type registry; section-level union), not routed through the path-level asset_preservation guard.
