Mission Run: Tool-surface projection honesty

Make 'doctor tool-surfaces --fix' and 'upgrade' actually repair every agent surface and converge in one run, instead of reporting success while leaving files stale.

Operators trust 'spec-kitty upgrade' and 'doctor tool-surfaces --fix' to bring an existing project's agent-facing files (orientation blocks, doctrine skills) up to date. Today both report success or partial progress while silently skipping surfaces they detected as stale (GEMINI.md never refreshes) or aborting mid-run so the same command must be re-run three times to converge (worst on Windows). This mission makes the repair path honest: it repairs every surface its own detector flags, converges in a single invocation, and reports a residual failure loudly rather than exiting 0.
