Mission Run: Fail-closed acceptance-matrix merge driver (#4880)
Make the acceptance-matrix merge driver fail closed on an unresolvable field conflict instead of silently corrupting a merged mission's verdict.
Today a routine git merge can embed conflict markers into acceptance-matrix.json and flip a mission's accepted verdict from pass to fail with no warning, undermining trust in mission history. This mission closes that verdict-corruption class by construction so acceptance verdicts either survive a merge intact or the merge stops for a human.
