Mission Run: Charter directive-resolution hardening

Harden the charter directive-resolution path: memoize repeated filesystem scans and warn when an unresolvable activation token is silently normalized.

Two robustness improvements to charter directive resolution deferred from the #4194/#4185 landing (epic #2519), neither changing resolution outcomes: cut repeated per-stem filesystem globbing, and surface a per-token diagnostic so a stale or mistyped directive activation entry no longer resolves silently.
