Mission Run: Tree-wide org-pack chain authority
Route every remaining org-pack chain-deriving caller through the single resolve_pack_chain() authority and widen the empty-allowlist gate tree-wide.
Follow-up to #6006, which established resolve_pack_chain() as the single ordered org-pack-chain authority and migrated the charter surfaces onto it behind a path-scoped empty-allowlist gate. This mission completes that unification across the ~12 remaining non-charter callers, widens the FR-006 census tree-wide with the allowlist kept empty, and completes #4984 fail-closed behaviour on the governed non-charter decision surfaces.
